Authorization Agreement

I hereby authorize (”Authorization”) ManifestHQ, Inc., ManifestHQ Advisors LLC, and affiliates (collectively, “Manifest”) to act as my agent on my behalf to collect, keep, and record all of my retirement account information. Specifically, Manifest has my Authorization to inquire on my behalf of any custodian or plan sponsor regarding the following information:

• presence of any of my retirement accounts at their institution;

• retirement plan custodian name and contact information;

• current account balances for each account;

• personal rate of return for each account;

• current fees being paid for each account;

• process for transferring those assets to another firm; and

• any other information as necessary or advisable to complete Manifest’s service on my behalf.

Manifest has my Authorization to present itself as my agent with any custodian or plan sponsor and to deliver to Manifest all of the information listed above as if the custodian or plan sponsor were interacting directly with me. Manifest has my Authorization to login into my custodian or plan sponsor’s digital portal on my behalf for the express intent in locating the above information.

It is my express intent that this Authorization be comprehensive as to its authority. I agree that my custodian or plan sponsor not require my presence physically or on a telephone call or require that I complete any additional forms to deliver the above information of my account to Manifest.

This Authorization is effective as of the date of the E-Signature herein and will expire upon the earliest occurrence of the following:

1. Manifest has conducted a full investigation of all my custodians and plan sponsors; or

2. Two months from the date of this electronic signature execution.
